Don’t Forget To Register With The State PA Medical Marijuana Program
Medical marijuana ID cards are issued by the Department of Health. You must register with the state using your driver’s license or photo ID number. Their system instantly verifies and gives you a MMP patient ID number.
At your appointment, our doctors will update your record to certify that you have been seen and approved for medical marijuana/cannabis treatment.
You are responsible to log back into the Department of Health medical marijuana patient registry and pay with a credit or debit card. This activates their system to create and ship your ID card in the mail.

Can a patient be under 18 or a minor?
Yes. A patient under the age of eighteen must have a caregiver who is approved by the department in order to obtain medical marijuana. A caregiver can be a parent, guardian or an individual approved by the department.
All applicants will need:

- Completed electronic copy of the Safe Harbor Act Letter Form
Legal guardians will need an electronic copy of their guardianship papers
- Caregivers will need an electronic copy of their caregiver status
- Spouses will need an electronic copy of their marriage certificate

I don't have any medical records. Can I still qualify?

What are Pennsylvania's qualifying conditions?
Pennsylvania law has a list of Qualifying Conditions. Our doctors certify that you have one of these and a Pennsylvania resident. The current list of conditions that can be treated by medical marijuana include the following qualifying conditions:
- Amyotrophic lateral sclerosis;
- Anxiety disorders;
- Cancer, including remission therapy;
- Crohn’s disease;
- Damage to the nervous tissue of the central nervous system (brain-spinal cord) with objective neurological indication of intractable spasticity, and other associated neuropathies;
- Dyskinetic and spastic movement disorders;
- HIV / AIDS;
- Huntington’s disease;
- Inflammatory bowel disease;
- Intractable seizures;
- Multiple sclerosis;
- Neurodegenerative diseases;
- Opioid use disorder for which conventional therapeutic interventions are contraindicated or ineffective, or for which adjunctive therapy is indicated in combination with primary therapeutic interventions;
- Parkinson’s disease;
- Post-traumatic stress disorder;
- Pain: Severe chronic or intractable pain
- Sickle cell anemia;
- Terminal illness; and
- Tourette syndrome
